Adult and Senior Services

Our goal is to provide compassionate, trusted care to enable seniors to live independently in their homes with respect.  We accomplish this through our individual Case Management, programs offered through our Multicultural Senior Center and our senior newsletter, The Urban Senior.

Visit our Multicultural Senior Center page for information on activities and programs offered through the Center.

Case Management

Our program continues to see positive outcomes that enable seniors to stay in their homes longer due to our strategic approach to serve our community. The plans that our case managers put into place and regularly monitor allows seniors to stay in their homes and neighborhoods that are familiar to them. They are able to continue to attend churches that they have grown to love over years and maintain friendships that they have acquired.

We get daily inquires about our services and provide services to over 800 individuals each year.  People access and are referred to our programs daily by social workers, hospitals, neighbors, family members or direct inquiries.

Seniors who access our services need assistance with:

  • Health Issues
  • Minor Home Repairs
  • Medical Equipment
  • Personal Care
  • Housekeeping
  • Transportation
  • Meals
  • Housing
  • Senior Law
  • Activities
